Cuff tear arthritis (CTA) is a combination of an irreparable large rotator-cuff tear with glenohumeral joint inflammation. The humeral heads are typically decentered to the superior side of the scapula in this condition.



A prosthetic glenoid is at risk of loosening due to the lack of centering provided by the normal rotator. 


In which the superiorly decentered head of the humeral bone differentially loads the superior side of the glenoid component and prys it away from its bone

A reverse total shoulder replacement (RSA) is one of the options available to treat disabling cuff tearing arthropathy. See this The link below will take you to the CTAH (see right). See this link).


    

The CTAH can be a good option for the active patient with cuff tear arthropathy and the ability to actively elevate the arm above the horizontal. You can see an example in this case (link). 

As shown below, the CTAH is not effective in patients with anterosuperior egress or pseudoparalysis.
